Function SummaryReporting to the Manager, Patient Registration and Flow, Be accountable for completing the pre-admission, triage and coordination for in-patient admissions in Complex Continuing Care, Rehabilitation and Palliative Care. Establish and maintain effective and efficient patient centered processes to ensure the timely admission of eligible patients in close collaboration with internal and external partners. Collaborate with team members across programs and sites as well as with other health care facilities across Ontario Health East.
Commit to living the Mission and values of Bruyère. Commit to patient and resident safety, on-going learning and continuous quality improvement. Required Qualifications- University degree and member in good standing of a regulated health profession
- 5 years of current or recent clinical experience in Complex Continuing Care, Rehabilitation and/or Palliative Care
- Advanced computer skills and advanced medical terminology skills
- Fluent in French and English– spoken and written (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement.)
Key Competencies- Advanced clinical assessment skills and clinical judgment as it relates to determination of eligibility of patients and admission planning process
- Strong communication skills and ability to interact with multiple agencies and individuals
- Advanced computer literacy for efficient frequent navigation of multiple different computer applications, including electronic patient record systems
- Strong organizational skills and leadership abilities
- Knowledge of the health care system
- Strong demonstrated ability to function in a self-directed manner
- Balance competing priorities in fast-paced environment according to system patient flow needs and bed availability
