Function SummaryReporting to the Director, Champlain Hospice Palliative Care Program, this role assists with the coordination of regional activities in advance care planning and other program areas as needed. This role plans and facilitates education and training sessions for a variety of stakeholders including clinicians, volunteers, patient organizations, group homes and community groups. This role is accountable for program planning, including organizing, facilitating, and evaluating activities based on strategic priorities in order to provide high quality program offerings. This role seeks to build regional collaboration between interconnected projects to create efficiencies as well as interacts effectively to bring diverse groups together.

Required Qualifications- Bachelor’s degree in social work or another health-related discipline or equivalent combination of experience, education and training
- Minimum five (5) years’ experience in a health care setting
- Progressive management experience
- Experience in the hospice palliative care system
- Fluent in French and English, preferred
