The ARC initiative aims to improve equitable Access to Resources in the Community for primary care patients. Patients seeking health services from their family physician often do not know what resources exist in the community that they could benefit from. Primary care staff may not know of the help that patients could receive from community resources for their health. Even after a patient has a referral, there may be limited access to appropriate resources, including French-language services for Francophones in Ontario. Additional barriers such as lack of transportation, social isolation or low literacy may also affect access. Sometimes patients need help navigating the system.
The ARC Patient Navigator works with primary care practices to help patients overcome barriers to access and use community health and social resources (e.g. healthy eating, physical activity, mental health or parenting support programs, etc.).The Navigator actively offers services in French for the Francophone population, and provides information, practical assistance, social support, and advocacy to help patients overcome barriers to access and facilitate the use of health enabling community resources. H/she serves as a link between patients, primary care, and community care through collaboration and effective exchange of information to promote navigation services and leverage community resources to assist patients.
Main duties and responsibilities:- Assist patients to identify and overcome barriers to access community health and social resources (CR) and utilize these services. The Navigator meets with patients in-person [at a primary care clinic, community health center, or mutual agreed upon public location] or by telephone to:
- Review the referral form completed by the patient and their provider recommending that the patient access one or more CR
- Assist patients in setting priorities related to their healthcare needs and developing a plan to enable access to recommended CR
- Identify social barriers that may stop patients from getting to the CR and engage patients in creating potential solutions to address these barriers
- Research community programs and services to identify appropriate and credible CR responsive to patient needs
- Provide assistance, support, and/or accompaniment to help patients reach the appropriate CR. (e.g. make appointments, coordinate transportation)
- Promote patients’ self-efficacy in navigating CR
- Empower patients to participate in their wellness by providing health-enabling and self-management resources
- Follow-up with patients to determine outcomes related to accessing CR
- Implement an “Active Offer” of French language services
- Communicate effectively with primary care providers, practice staff, patients and community agencies for effective navigation services
- Demonstrate sensitivity and responsiveness to a diverse patient population
- Participate in and promote on-going opportunities to support the provision of culturally relevant services to Francophone patients
- Document patient encounters, maintain accurate and complete records of navigation services provided
- Provide timely feedback to primary care providers about patients’ participation in community resources
- Share experiences and best practices with other Navigators
- Work with other Navigators to improve how patients can be helped to reach community resources
As a member of the ARC research team: - Collect data for research and tracking purposes throughout the study
- Communicate regularly with the ARC Coordinator to collaboratively improve navigation process
- Engage in ongoing learning and use new knowledge and skills to promote navigation role
- Support the research team with the development and adaptation of study material (e.g., guides, methods for data tracking and data collection, etc.)
- Other duties as reasonably assigned
Qualifications:- Training, a degree, or diploma in health, social services or psychology or experience in a relevant field
- Minimum of two years’ experience in a community setting in health or social services
- Experience working with Francophone community and culture
- Knowledge of basic principles and practices of community service delivery
- Proven ability to work both independently and with other people in solving complex problems
- Ability to be open and non-judgmental, respecting individual preferences and choices
- Skilled at critical thinking to develop and evaluate options and implement solutions
- Skilled at planning and setting priorities
- Ability to apply outreach methods and strategies to link primary and community care
- Fluency in French (written & oral) and competency in English
- Proficiency in computer skills including Microsoft Office, internet search engines
- Proficiency in electronic medical record (EMR) software an asset
- Knowledge of Healthline, Ontario 211 websites an asset
- Ability to work flexible hours based on patient and primary care practice needs
- Ability to travel to community organizations and primary care clinics as needed
Some travel will be required for this position to meet patients within the boundaries of the Champlain Local Health Integration Network.
