Function summaryReporting to the Director, Family Health Services, the incumbent provides psychosocial assessment of clinic patients; advises physicians of the psychosocial context of the health problem and provides help in resolving psychosocial issues that are having an impact on health; ensures that patients are connected to the appropriate community, volunteer and professional services and programs; coordinates appropriate care and patient education; provides individual and group sessions to patients in the understanding and living with particular health issues; and provides information to other health care providers and facilitates a seamless transition between care settings. The incumbent also provides teaching to learners in the Family Medicine Centre.

He/She is committed to patient and resident safety, on-going learning and continuous quality improvement. Required qualifications- Master's degree in Social Work (MSW) from an accredited university
- Membership in the Ontario College of Social Workers and Social Service Workers
- Fluent in French and English
- Minimum of three (3) years experience as a Social Worker with patients in a health setting
- Experience in clinical counseling/psychotherapy (individual, couple and family counseling, mental health, crises intervention, etc.)
Preferred qualifications- Demonstrated competence in team building, conflict management, and interpersonal effectiveness
- Excellent oral / written skills
- Strong clinical skills
- In depth knowledge of chronic illness care, including both clinical and supportive care
- In depth knowledge of the local health program delivery and community based services
- Ability to develop and maintain networks at the local and provincial level
- Ability to assess human bio-psychosocial responses
- Able to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population
- Strong group presentation skills
- Flexible
- Autonomous
- Ability to work effectively with all members of the health care team
- Ability to work in an inter-professional team environment
- Ability to prioritize, manage time effectively and be flexible in a very active work environment
