Function SummaryReporting to the Director, Pharmacy and Laboratory Services, the incumbent conducts individual patient assessments, provides recommendations and follows up on medication usage; provides patient education regarding medication use and collaborates with the interprofessional team to improve the medication management program for better patient care. This position will be responsible for providing care to a Palliative Care population.
He/She is committed to living the Mission and Values of Bruyère Continuing Care. He/She is committed to patient and resident safety, on-going learning and continuous quality improvement. Required Qualifications- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD degree)
- Licensed with the Ontario College of Pharmacists
- Hospital pharmacy residency or equivalent clinical experience
- Advanced interpersonal, organizational and communications skills
- Ability to work in a inter-professional team environment
- Ability to prioritize, manage time effectively and be flexible in a very active work environment
- Demonstrated proficiency providing direct care in a Pharmaceutical Care model
- Ability to communicate in both official languages, spoken and written
