Function Summary

Reporting to the Clinical Manager, Stroke Rehabilitation, this role:

Supports the Outpatient Stroke Rehab service delivery model and is responsible for delivery of high-quality outpatient stroke rehabilitation therapy, triaging referrals and coordinating patient access to stroke rehabilitation services.

Maintains appropriate patient flow, provides oversight for the day-to-day operation of the program as well as collaborates with the Clinical Manager and physician leadership on the planning, organization, direction, and evaluation of outpatient stroke rehab services in keeping with the needs of the clientele in accordance with the best practice guidelines.


Is responsible for:

 
Day-to-day clinic operations
Overseeing the Outpatient Stroke Rehabilitation clerk as a direct report
Assisting with staff scheduling and supervision, including providing input to biannual performance reviews
Organizing on-boarding for new employees
Environmental, safety and supplies of the program
Adherence to Bruyère policies and procedures
Clinic systems management
Navigating patients through the outpatient stroke continuum of care
Acting as a resource to staff, physicians and patients
Liaising with the Champlain Regional Stroke Network and other health care partners
Managing patient complaints.

Required Qualifications

  • Undergraduate degree (4 years) in Nursing, Occupational Therapy, Physiotherapy, Speech and Language therapy or Social Work.
  • Member of a Health Professional College regulated under the Regulated Health Professionals Act of Ontario.
  • Four (4) years of experience in a related field.
  • Working knowledge of the health care system.
  • Advanced computer skills and medical terminology.
  • Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement).
