Transitional Care

Preparing for Discharge

Discharge is an exciting time, but it can also be overwhelming. When your application to Long-Term Care is accepted, or a discharge date is determined if going to the community, you will be discharged from the hospital. You may feel anxious about leaving the hospital – a place where you are comfortable and have built relationships. Your safety is our priority. We recommend discharge only when we are confident you are ready.

 

A member of our Quality, Patient Safety and Risk Management department will contact you 24-72 hours after your return home to obtain feedback on the quality of our discharge planning process. During our call, we will also be available to answer any questions or concerns you might have related to your discharge, or guide you to the right individual or resources if needed.
