Transitional Care

Your Care Goals

The transitional care unit is a place of transition, not a destination. Your care team is there to provide support while you heal and transition home or to another care setting. We need your help to identify your care goals, and for you to participate and be engaged in all aspects of your care plan.


Here are some ways you and your family can be prepared:

  1. Get the equipment you need for your daily activities. Your Physiotherapist and Occupational Therapist will assess your needs and help you order your own equipment to ensure your successful transition. If needed, there are programs to assist with covering the costs of these supports.  
  2. If discharge to a long-term care facility is your plan, work with your care team and the Home and Community Care Support Services to complete the long-term care application. You will also need to arrange your transportation to the long-term care location.

Your Social Worker and the Home and Community Care Support Services can help you along the way.
