Function Summary

Reporting to the Manager, Patient Registration and Flow, this role is accountable for completing the pre-admission, triage and coordination for in-patient admissions in Complex Care, Rehabilitation and Palliative Care. 

 
Establish and maintain effective and efficient patient centered processes to ensure the timely admission of eligible patients in close collaboration with internal and external partners.


Collaborate with team members across programs and sites as well as with other health care facilities across Ontario Health East.

Required Qualifications

  • University degree and member in good standing of a regulated health profession
  • 3-5 years of current or recent clinical experience in Complex Care, Rehabilitation and/or Palliative Care 
  • Extensive (or in-depth or superior) knowledge of the various diseases with associated sequelae as well as treatment modalities with associated side effects
  • Superior knowledge on the progression of chronic diseases as it would relate to a patient's eligibility for admission to Complex Care, Geriatric Rehabilitation, Stroke Rehabilitation and Palliative Care programs
  • Demonstrated ability to perform advanced clinical assessments using evidenced based knowledge
  • Strong decision making skills
  • Ability to work in a fast paced and at times stressful work environment through task prioritization and strong organizational skills
  • Intermediate computer skills and advanced medical terminology skills
  • Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement.)

  • Exhibits strong interpersonal and leadership abilities as evidenced through the ability to professionally work through situations of conflicts
  • Knowledge of the health care system as it relates to patient eligibility for services for the Complex Care, Geriatric Rehabilitation, Stroke Rehabilitation and Palliative Care populations
  • Demonstrates an openness to embrace new technologies to improve day to day efficiencies
