Uninsured services 

 

The Ontario Health Insurance Plan (OHIP) provides coverage for a vast majority of physician services. However, some services are not covered; talk to your physician about it for more information.

 

 

Quality health care, medical advice and timely access to services are priorities for Ontario’s physicians. Although physicians are pleased to offer the following valuable services, the payment is the responsibility of the patient or of the third party requesting the service.

 

 

Medical services:

  • - Examinations or diagnostic tests necessary to satisfy employers’ requirements
  • - Insurance companies, government agencies, etc.
  • - Pre-adoption examinations
  • - Travel medicine advice and other services for travelling outside of Canada
  • - Cosmetic procedures and related examinations and diagnostic tests

Completion of forms and medical records:

 

  • - Medical/legal reports
  • - Federal government forms
  • - Certificate for the purpose of travel, immigration, employment, etc.
  • - Driver’s/aviation medical exam and other licensing forms and certificates
  • - Parking forms
  • - Insurance forms, reports and certificates, such as:
  •          - Disability certificate
  •          - Treatment plan
  •          - Insurance medical examination reports
  • - Completion of work and school-related forms and notes, such as:
  •          - Sick or back to work notes
  •          - Admission to daycare, school, recreational programs
  •          - Reports and forms requested by employer (e.g., form physicals)

Forms are important to us and will be completed as soon as possible. Payments can be made by checks, e-transfer finance@fmao.ca and at the clinic with all major credit cards.

 

Family Health Organization

Ontario patients with OHIP coverage are asked to formally join our Family Health Organization (FHO) by signing an agreement with their family physician here at our Centre. Enrolling with the FHO gives you access to the many services offered by our Family Health Team and after-hours care.

 

Results

The great volume of test results makes it impossible to call patients with every normal result. You will, of course, be informed of significant abnormal results, and if we feel the results require discussion, we will ask you to come in. If you wish, you may make an appointment to review your results.
