Reporting and Request Forms 

Please allow at least two weeks for the processing of all requests.

Please note: For all research, you are required to submit all modifications and other reporting via the Cayuse platform.

Modifications (previously Amendments) 

For all modifications, please submit the Amendment Form, along with all requested changes clearly outlined within the BREB, protocol, consent forms, advertising materials, etc.

All revisions must be made using track-changes, and you are required to submit both the track-changes and clean versions of all documents.

For the addition of new staff that will require access to the Cayuse platform, please submit the Amendment Form, a signed pledge of confidentiality, and a TCPS2 certificate (issued within the last five years).

Study Renewals

Studies receive REB approval for one year only and must be renewed every 12 months to ensure continued ethical approval. It is the responsibility of the researcher to ensure the study is renewed in a timely manner. If the study is not renewed, the REB may close the study, and a new application must be submitted if the study is to be continued.

Please submit all renewals via the Cayuse platform.

Study Closures

Studies may be closed if the following conditions are met:

• Data collection is complete, and you are certain no more data collection will occur.
• All participant contact has ended (this includes follow-up visits/close-out correspondence with participants, etc.).
• Study sponsor has issued a study closure (for clinical trials).
• Data analysis is complete.
• Manuscript has been accepted or published by a journal (this is strongly encouraged, but not necessary for study closure).
