Consent to treatment

By accepting admission to one of our residences, you agree that our staff may provide you with basic care as described in the Resident Contract. You and your family have a right to be involved in the decision-making process on an ongoing basis. You and your family have the right to make informed choices about the interdisciplinary care plan and suggested treatment. We will seek consent from you or your power of attorney for care in advance of any new treatment.

To ensure your safety, all staff must check at least two resident identifiers (such as name, date of birth, assigned identification number or photograph) prior to any assessment, treatment or service (including blood work, specimen collection and medication administration).
